Job Qualifications and Experience Minimum or Required :
Bachelor of Science Degree in Pharmacy or entry level Pharm. D.
A valid license to practice as a pharmacist issued by the Saudi Council for Health Specialties.

Job Purpose Provides pharmacy services to assigned areas, including preparing, compounding, and dispensing medications to patients according to the policies and regulations. Work relatively independently, under general supervision, and within the limits of established pharmacy policies and procedures, as a member of the patient care team.
Key Accountabilities & Responsibilities
Responsible for preparing, compounding, and dispensing medications to patients according to the policies and regulations.
Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug related problems and improving therapeutic outcomes.
Work relatively independently, under general supervision, and within the limits of established pharmacy policies and procedures, as a member of the patient care team.
Oversees the activities of support personnel, operating within the limits of pharmacy / practice site’s policies and procedures.
Direct and coordinate efficient and effective delivery of all pharmacy services.
Interpret, clarify, and evaluate all medication orders for complete and appropriate medication information, scheduling, and clinical data.
Provide drug information and consultation to physicians, nurses and other health care professionals regarding appropriate drug therapy, including drug interactions, side effects, incompatibilities, allergies, medication order / prescription discrepancies, drug stability problems, and drug administration.
Identify, document, resolve, monitor, and prevent drug therapy related problems, including contraindications, duplications of therapy, and drug interactions.
Work in the sterile products area as needed, ensuring the accuracy, sterility, and stability of all compounded products and provide feedback and suggestions to optimize workflows.
Formulate and compound special dosage forms not commercially available when appropriate.
Maintain appropriate records for controlled substances and investigational drugs as required and provide feedback and suggestions to optimize workflows.
Provide discharge medication and drug history counseling including medication reconciliation when appropriate and develop teaching tools and guidance for staff.
Support research-related patient care and drug therapy by ensuring study protocol medication order compliance.
Train new personnel.
Interpret and implement all departmental / practice site policies and procedures on the assigned shift.
Follow all policies, procedures, and regulations set forth by KAAUH and all other applicable regulatory agencies.
Ensure compliance with target drug programs, formulary management, medication management initiatives, and adverse drug event monitoring programs.
Document and report all clinical interventions, medication errors, near-misses, and adverse drug events.
Participate in the preparation of drug administration guidelines, medication and disease management guidelines, or other guidelines for medication use.
Utilize area-specific technology, including, but not limited to, automation, software, equipment, and information systems when appropriate.
Report short-term issues related to the area specific technology.
Participate in departmental quality improvement committees, programs, and student / resident presentations.
Attend and participate in departmental staff meetings, and staff development and continuing education activities.
Attend and participate in grand rounds and continuing education activities.
Maintain professional expertise through membership and participation in professional organizations and contributing to peer-reviewed literature.
Prepares and presents educational materials that promote the safe, effective and rational use of medications within KAAUH.
Works as per work schedule including shifts & on-call duties as assigned.
Prepares reports related to medication use at KAAUH as required by the immediate manager / Director of Pharmacy Services.
Serves on KAAUH committees as requested.
Respond with the code blue team, in case of CPR when scheduled.
Work in rotating shifts or per site’s needs where applicable (which may include weekends, holidays and on-call responsibilities).
Respects patients and their families to promote a patient-centered care culture.
Performs other related duties as required.
