To participate in the inpatient services management team to implement the pharmaceutical care concept of practice. To Plan, develop, implement and monitor clinical projects, and / or programs in the different pharmacy service’s areas as it relates to clinical practice. To serve as a pharmaceutical caregiver by efficiently designing, recommending, activating, monitoring, and evaluating patient-specific pharmacotherapy to ensure optimal outcomes. To serve as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other health care professionals Key responsibilities of the role : Contribute to developing pharmacy service standards and ensure they are consistently met. Act as a role model and deliver high clinical and operational performance standards by developing and supporting the Pharmacy Team. Collaborate with nursing and allied health staff at all levels, interdisciplinary teams, executive officers and other stakeholders in the development, implementation, and evaluation of programs and services. To market, promote and profile the service to patients, referrers and the general public and to undertake business development activities. Ensures the reporting of all adverse events, incidents and near misses. Contributes to investigations of complaints and incidents and draft responses as required in a timely manner. Contributes to the monitoring of key performance indicators and developing action plans as required Processes medications orders (example : medication order entry and verification) Identifies and capable of using resources to ensure safe medication practices. Responds to incidences, adverse drug reactions, medication errors, and reports appropriately. Checks all prescriptions / physicians orders for accuracy before dispensing. Checks pre-packing, ward stock, pharmacy trays (and any other pharmacy kits) prepared by pharmacy support staff. Dispenses medications to patients / nurses. Counsels patients / nurses with clear and accurate instructions (either by verbal counseling, written labels or MARs), ensuring understanding of instructions. Calculates and confirms pharmaceutical dosage calculations (pediatric doses, IV admixtures, TPN, etc.). Removes and replaces expired medication. Performs patient care area / clinic ward stock inspections. Actively participating in patients care rounds with other members of the health care team. Developing clinical protocols / guidelines that relate to clinical practice Assisting medical staff with the selection of drug therapy and patient monitoring Reviewing and interpreting medication orders for clarity, accuracy, rationality, compatibility, feasibility, and suitability for the patient’s specific needs. Being responsible for patient’s specific pharmacotherapy plan to ensure optimal outcome. Determining workload priorities, verifying medication orders, checking the final product for accuracy, and assuring timely delivery of the medications to the patients care area. Acting as drug information and education resources for patients, students, colleges, medical and nursing staff and other health care professionals Documenting therapeutic recommendations in the patient’s medical record and pharmacy intervention forms Detecting, analyzing, and reporting medication errors and adverse drug reactions Assuring adherence to the departmental policies and procedures Providing a monthly report on clinical activities and other relevant workload statistics Attending assigned committee Participating in departmental performance improvement, educational and other clinical activities Assisting in evaluating drug for addition or deletion to the formulary Conducting drug utilization reviews Participating in training programs for pharmacy staff, residents and interns Encourage the spirit of teamwork and seeks to form a healthy environment through pharmacy Performing patients and family education as needed Following all hospital related policies and procedures Perform other applicable tasks and duties within the realm of his / her knowledge, skills and abilities Participates in quality management activities as assigned and at a personal level of accountability, i.e. accreditation process. Evaluates factors related to safety, outcomes, effectiveness, cost, and social impact when developing and implementing practice innovations. Participate in the Hospital’s Quality and Audit Programmers and evaluate the impact of new practices. Provide education and counseling to patients and caregivers regarding proper medication use, potential side effects, and adherence strategies. Participate in the development of continuing education programs and clinical guidelines related to pharmacology and therapeutic management. Precept and mentor pharmacy interns, residents, and junior staff in clinical practice and research activities. Participates in the formulation of Quality Assurance programs in line with strategic direction. Participates on relevant committees and on special projects as required. Maintains confidentiality regarding patient / staff information. Ensures all allied health staff maintains confidentiality regarding patient / staff information. Functions as a clinical resource to all Staff and acts as liaison with other disciplines and departments. Promotes positive public relations with patients, peers, medical staff and members of the general public. Collaborates with personnel from non-clinical department / disciplines regarding issues that may negatively impact patient care; facilitate solutions as needed. Maintain a positive work environment for staff and promote team efforts, promote and maintain effective working relationships with all levels of staff. Adheres to the policies of Occupational Health and Safety and Infection Control Guidelines in all work practices. The position does have heavy exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents. The position requires high exposure to infectious wastes such as blood and body fluids that mandate the wearing of gloves, masks and goggles for every actual or potential exposure. Emotionally self-aware, practice self-control, able to empathetically interact and understand the team and their needs and demonstrates effective management of relations to ensure team success and better patient care outcome. Acts as a role model for all staff. Maintains a personal record of continuing education attendances, (direct or indirect). Actively participates in the annual system of performance revie Qualifications, Certifications and Experience : Master's in clinical pharmacy or SSC-Ph.P. Valid license / registration to practice his / her specialty from Saudi Commission for Health Specialties. BLS certification. Knowledge of KSA Health Regulations. International accreditations experience
