SUMMARY

The Physical Therapist is a licensed professional who directs,supervises, evaluates and provides physical therapy services to thepatients as prescribed by the attending physician. May supervisePhysical Therapist Assistants and Aides.

QUALIFICATIONS

Graduate from anaccredited physical therapy school. Preferably two years ofclinical experience (if applicable). Capable of lifting theequivalent weight of an adult-size patient. Licensed by the statein which the Physical Therapist resides or practices PhysicalTherapy according to staterequirements.

PHYSICALDEMANDS

Requiresmoderately heavy physical exertion on a regular and reoccurringbasis such as extensive driving, assisting patients in transferactivities (wheelchair to bed, to tub, to commodes) and providingsubstantial support to individuals in ambulation therapy. Regularlyrequired to use hands and arms. Frequently required to stand, walk,reach with hands and arms, stoop, kneel, crouch, or crawl.Occasionally required to sit. Occasionally lift and / or move over100 pounds. Specific vision abilities include close, peripheral,depth perception and ability to adjust focus.

WORKING CONDITIONS

General office, health care setting - exposure to infectiousdisease.

ESSENTIALFUNCTIONS

1. Provides treatment as ordered bythe attending physician. Interprets, plans, establishes, andexercises proper treatment program in accordance with the physicians referral, periodically, reviewing the patient s program andresponse and making recommendations to the physician relative tothe need of continuing or discontinuing the physical therapytreatment. Assists the physician in evaluating disability, injury,or disease process of individual patients and in evaluating thepatient s level of function.

2. Contacts thephysician directly for patient reports and care as needed.Observes, records, and reports to the physician the patient sreaction to the therapy program or any changes in the patient scondition. Submits to the physician and the Program Supervisorappropriate written reports to the patient s progress at thedesignated intervals using appropriate and designatedforms.

3. Assist in determining the need ofthe patients an in developing the total plan of care.

4. Instructs, teaches and supervises patients regardingphysical therapy procedures as related to the patient s therapyprograms.

5. May supervise PhysicalTherapist Assistant(s) and co-sign Physical Therapists Assistant sprogress notes.

6. Adheres to all applicablefederal, state, local and company maintained standards of care andethics and policies concerned with the practice of physical therapyand among other things, personnel qualifications.

7. Participates in staff development activities and inin-services education programs.

8.Demonstrates evidence of continuing education conducted forin-service sessions.

9. Meets productivityand utilization expectations as established by the Company and thefacility.

10. Completes timely clinicaldocumentation to include functional goal-setting, progress andtherapeutic approach.

11. Completesadministrative / billing documentation timely andaccurately.

12. Covers more than onefacility as required by the need of the Company.
