Pharmacist

To provide pharmaceutical expertise and input to the care of patients and provide patients and clinical providers with therapeutic assessments and selected disease management, whilst working toward improving patient and clinical practice outcomes. The Pharmacist will ensure the delivery of international standards of care in accordance with established National and International practices, policies and procedures. Key Accountabilities & Responsibilities : Contribute to developing pharmacy service standards and ensure they are consistently met. Act as a role model and deliver high clinical and operational performance standards by developing and supporting the Pharmacy Team. Collaborates with nursing and allied health staff at all levels, interdisciplinary teams, executive officers and other stakeholders in the development, implementation, and evaluation of programs and services. To market, promote and profile the service to patients, referrers and the general public and to undertake business development activities. Ensures the reporting of all adverse events, incidents and near misses. Contributes to investigations of complaints and incidents and draft responses as required in a timely manner. Contributes to the monitoring of key performance indicators and developing action plans as required Responsible for the implementation and delivery of age-appropriate patient care congruent with the mission, vision and values of the organization. Work with patients who have a range of conditions, sometimes over a period of weeks. Reviews medication orders and patient medication profiles for appropriateness of therapy. Recommends changes in medication therapy based on appropriateness and medication management initiatives (example : policies). Monitors for drug interactions, duplication of therapy, allergies, adverse reactions, appropriate formulary alternatives and identifies medications. Processes medications orders (example : medication order entry and verification). Maintains proper documentation on patient profiles or prescriptions. Identifies and capable of using resources to ensure safe medication practices. Responds to incidences, adverse drug reactions, medication errors, and reports appropriately. Checks all prescriptions / physicians ordersfor accuracy before dispensing. Checks pre-packing, ward stock, pharmacy trays (and any other pharmacy kits) prepared by pharmacy support staff. Counsels patients / nurses with clear and accurate instructions (either by verbal counseling, written labels or MARs), ensuring understanding of instructions. Calculates and confirms pharmaceutical dosage calculations (pediatric doses, IV admixtures, TPN, chemotherapy, etc.). Removes and replaces expired medication. Performs patient care area / clinic ward stock inspections. Maintain an active caseload as required by service demand. Operates independently under broad direction to promote a co-operative and effective environment for patient care within the facility. Demonstrates clinical expertise in specialty. Perform other applicable tasks and duties within the realm of his / her knowledge, skills and abilities Utilises evidence-based practice to develop and implement standards that guide practice improvement initiatives. Participatesin quality management activities as assigned and at a personal level of accountability, i.e. accreditation process. Evaluates factors related to safety, outcomes, effectiveness, cost, and social impact when developing and implementing practice innovations. Participate in the Hospital’s Quality and Audit Programmes and evaluate the impact of new practices. Requirements : Bachelor’s degree in pharmacy from one of the recognized universities. Valid license / registration to practice his / her specialty from Saudi Commission for Health Knowledge of KSA Health Regulations. International accreditations experience an advantage Minimum of 3 years as a hospital pharmacist within an acute care hospital. Seniority level
