The Clinical Pharmacist – Outpatient Care ensures safe, effective, and patient-centered medication use in ambulatory care settings. The role includes direct clinical services, medication management, and collaboration with healthcare team members to enhance therapeutic outcomes, promote adherence, and ensure continuity of care.
Patient-Centered Clinical Services
- Conduct comprehensive medication therapy management (MTM) for chronic disease patients.
- Provide face-to-face or virtual patient education and adherence support.
- Adjust and optimize medication regimens under collaborative practice protocols
Medication Review & Safety
Perform medication reconciliation during care transitions.Identify and resolve drug therapy problems and prevent adverse drug events (ADEs).Ensure appropriate documentation and reporting of ADEs and interventionsCollaboration & Care Integration
Participate in clinical case reviews, huddles, and interprofessional care planning.Support chronic disease clinics and contribute to guideline-based care.Coordinate follow-up plans in collaboration with physicians and care teams.Operational & Quality Support
Ensure compliance with formulary guidelines, policies, and documentation standards.Participate in cost-containment efforts and therapeutic interchange programs.Contribute to policy development and clinical pathway updatesEducation & Development
