Case Manager

About the Job

We are currently seeking a compassionate and experienced Case Manager to join our team in Abha, KSA .

As a Case Manager, you will be responsible for coordinating and managing patient care within a Rehabilitation and Long-Term Care (LTC) facility.

The Case Manager ensures that patients receive appropriate, efficient, and high-quality care through effective coordination, discharge planning, and communication across multidisciplinary teams.

This role plays a key part in promoting optimal patient outcomes and ensuring continuity of care in compliance with healthcare standards and regulations.

Job Requirements

• Bachelor’s degree in Nursing (BSN) or a related healthcare field — mandatory .
• Must hold a valid professional license from the Saudi Commission for Health Specialties (SCFHS) .
• Minimum of 3–5 years of clinical experience , preferably in :
• Long-Term Care and Rehabilitation settings.
• Admission, Discharge Planning , or Care Coordination roles.
