Diabetic Educator

Job Purpose / Objective

Perform diabetes care management and serve as a diabetes education specialist for HMG. Provide expert clinical leadership, education and consultation to nursing and other disciplines in order to meet the complex needs of patients with diabetes and their families.

Key Responsibilities / Accountabilities

• Identifies learning needs of patients through recognized assessment method and assures quality of care in the clinical area.
• Sets objectives for each area where patient education is required.
• Provides ongoing assessment and recommendation to the treatment team and the patient / family for adjustments in the plan of care as required.
• Provides education related to diabetes and the disease process, medication, diet, and nutritional monitoring, foot care, identification of signs and symptoms of diabetic complications, prevention and care of complications, blood glucose monitoring and the techniques of self-monitoring and injection.
• Assesses, monitors and implements treatment plans for patient at various stages of the disease focusing on prevention of complications and management of stable diabetes.
• Documents assessments, interventions and plans including recommendations and goals, progress notes and discharge summaries.
• Participates in the development, planning and evaluation of treatment, education, counseling, and health promotion activities of HMG.
• Communicates effectively with all patients, families, peers, other health care professionals and community partners.
• Develops and maintains positive working relationships with all staff in the department and facility.
• Utilizes information from individual patients to identify broader implications for health within the community.
• Identifies key performance indicators for nursing services, participates in the measurement of these indicators.
• Demonstrates professional and educational ethics related to medical practice and patient confidentiality.
• Visits to venues outside HMG to educate public and enhance their awareness about Diabetes management.
• Provide patient and family education as per the needs.
• Participate in person-centered care initiatives undertaken by HMG.
• Enrich patient experience with compassion, respect and dignity.
• Performs other applicable task and duties assigned within the realm of his / her knowledge, skills and abilities.
• Education / Professional Qualification

Bachelor’s degree / Diploma or its equivalent

Professional - Preferably Nursing / Health Education

Minimum of at least 4 years working experience in same specialty

Valid registration of Saudi Council
