2.1 Responsible for the provision of nursing care that meets PSMMC Competency Standards for Registered Nurse, including individual accountability for action within the scope, practices and framework for Registered Nurse in accordance with PSMMC Policies and Procedure.
3.0 Duties And Responsibilities :
3.1 Clinical
Deliver and maintains safe quality nursing care according to PSMMC policies and procedures.
Utilize evidence-based approach in assessment, planning, implementation and evaluation of patient care.
Recognize and act on changes in patient condition, making safe, timely decisions appropriate to the situation.
Assist team leaders and members in liaising between healthcare professionals, physicians and allied healthcare professionals to ensure appropriate medical care is provided to all patients.
Ensure that appropriate documentation e.g. consent medical assessment and others is obtained and maintain for patients undergoing relevant medical treatment. Respond to life-saving situation following the appropriate nursing standards and protocols to treatment.
Ensures safe administration and management of medication after passing the
competency.
Apply knowledge and operates safely in handling specialized and technological equipment.
Advocate on behalf of their patients to ensure quality healthcare services are provided. Maintain professional interpersonal relationships with patients, families and coworkers / other members of the health care team.

Maintain privacy and confidentiality of all patient related information in relation to service provision and healthcare treatments and services.

3.5 Unit Specific Additional Duties and Responsibilities
3.6 Psychiatry
Conducts psychiatry nursing and risk assessment focusing on the needs of the patient and primary teams management issues.
Taking past and present medical psychiatric history of the patient through interview to obtain patient insight into the present problem and determine the need for short term treatment.
Conduct mental state examination.
Risk assessment and management of potentially aggressive and violent patients.
Prioritize patient intervention and carries out prescribed treatment as requested from the primary treating team.
Coordinate with Psychiatrist, Social Worker and Psychologist in the evaluation of treatment and planning.
Conduct 1 : 1 monitoring of patient who needs to be in safe condition in an environment that has minimal external stimulation especially to severely disturbed and suicidal patient.
Provide ongoing advice, monitoring and education regarding the management of mental health disorders, and recommended treatment plan within the psycho educational policy.
Attend the management and ward rounds on a regular basis and relays all information (e.g. new order, changes, modification) to the nursing team.
3.7 Intensive Care Services (ICS)
Provides direct and individualized total nursing care based on the application of scientific nursing principles, monitors and maintain standards of care within the Medical City Wide and ICU Policies and Procedures.
Assists team leaders and members in liaising with other health care providers to ensure appropriate treatment is provided to ICU patients.
Assists physician for any invasive and non-invasive bedside procedures and adhering to PSMMC policy and procedure on Aseptic Technique.
Ensures that IntellScape Critical Care and Anesthesia (ICCA) programs and critical care scoring are carried out, and properly documented.
Ensures that specialized ICS equipment and other types of medical equipment are functioning properly.
Knowledge on safe handling and operation of specialized and technological equipment.
3.8 Rapid Response Team (RRT / Medical Evacuation (MedEvac) Nurse)
Respond in a timely manner during emergency RRT calls.
Ensure patients clinical data e.g. bloods, radiological investigation and others are available.
Assists physicians in patient intubations and extubations.
Ensure that RRT emergency drugs are complete and available.
Maintain the completeness of the RRT / MedEvac trolleys.
Monitor all mobile patients condition and treatment.
Ensure that all documents are in order and complete including equipment before collecting MedEvac patients.
3.9 Pediatric Intensive Care Unit (PICU)
Assists in life-saving advanced resuscitation and invasive interventions.
Demonstrate skills and good judgment to quickly assess and treat critically ill child.
Utilize advanced knowledge and skills in assessing, planning, implementing and evaluating programs of care for pediatric patients in PICU from admission to discharge or transfer to lower level of care.
Ensure safe calculation, administration and titration of multiple and complex IV infusions working within prescribed parameters to achieve precise aims (i.e. Inotropes, Sedations, Patient Care Analgesia (PCA), Antiarrhythmic, Heparin, etc.) including a working knowledge of the drug effects, and action in the event of an adverse response.
Delivers highly proficient clinical skills in assisting and performing nursing care as per PSMMC policies and procedure after completion of PICU specific competencies :
3.10 Neonatal Intensive Care Unit (NICU)
Attend high risk deliveries and other emergencies as Neonatal Resuscitation Provider (NRP), ensuring that neonates receive quality, safe and effective advanced resuscitation.
Deliver excellent nursing care, monitor, maintain standards of care for neonates with invasive and non-invasive ventilation support.
On completion of relevant competencies pertaining to NICU, performs nursing care including calculate preparation, administration, document and evaluate the effects of drug and fluid therapies in accordance with PSMMC policies and procedures.
Initiate blood and blood products transfusions and volume expanders as clinically indicated and ordered, in accordance with PSMMC policies and procedures.
Participate and assist in performing the advanced procedures as ordered for critically sick ill and premature neonates after accomplishment of NICU general competencies and unit specific competencies
3.11 General Pediatrics
Deliver quality nursing care standard for all medical / surgical pediatric patients from age one (1) day to fourteen (14) years old.
Perform a thorough pediatric physical assessment, planning, implementation and evaluation is provided to all patients on a timely manner. Participate in the stabilization, transfer and admission of the extremely ill pediatric patients to or from the PICU.
3.12 Support Services :
3.13 Stoma Care Nurse
Plan, coordinate and implement the overall Stoma Care activities in accordance with the mission, vision and objectives of the institution in the assigned nursing units.
Initiate an evidence-based approach in the assessment, planning, implementation and evaluation of patient care.
Develop and implement stoma care protocols, documentation tools and referral criteria.
Identify and initiate the appropriate treatment plan on changes in-patient stoma condition, making safe, timely decisions appropriate to the situation in collaboration with the attending physician, nursing staff and multidisciplinary team.
Advocate on behalf of patients to ensure quality healthcare services are provided.
Prepare and maintain reports on a monthly and yearly basis, collect statistics and research details for the Stoma Care Committee.
3.14 Wound Care Nurse
Plan, coordinate and implement the overall wound care activities in accordance with the mission, vision and objectives of the institution in the assigned nursing units.
Initiate an evidence-based approach in the assessment, planning, implementation and evaluation of patient care.
Develop and implement wound care protocols, documentation tools and referral criteria.
Identify and initiate the appropriate treatment plan on changes in-patient wound condition, making safe, timely decisions appropriate to the situation in collaboration with the attending physician, nursing staff and multidisciplinary team.
Assist Team Leaders and members in liaising between healthcare professionals, physicians and allied healthcare professionals to ensure appropriate wound care is provided to all patients.
Assess wound and applies Negative Pressure Wound Therapy (NPWT) Machine if needed and other wounds modality.
Advocate on behalf of patients to ensure quality healthcare services are provided.
Ensure that the assigned units is equipped and maintained with adequate dressing supplies for the need of the patients.
3.15 Patients Advocate Nurse
Discuss the living donation process in general terms to include donor evaluation, donation surgery, post-donation recovery, potential complications and follow up.
Discuss with the donor their current medical history and its implications for the suitability of the donor.
Identify the current and future risks for the living donor using a multi-faceted framework and not solely a medical model.
Discuss the emotional / psychological aspects of living donation.
Understand the importance of a psychosocial assessment to evaluate external pressures that may impact the donation decision.
Explain the importance of family support system regarding the donation decision and the risk of trying to proceed without the needed support.
Ensure that the donor makes the decision free of coercion and pressure to donate.
3.16 IV Nurse
Plan, coordinate and implement IV nursing care in accordance with the PSMMC policies and procedures.
Evidence-based approach to intravenous assessment, planning, implementation and evaluation of patient care.
Assist Team Leaders and members in liaising between health care professionals, physicians and allied health care professionals to ensure appropriate IV care is provided to all patients.
Identify and act on changes on patient condition, making safe, timely decisions appropriate to the situation.
Advocate on behalf of their patients to ensure quality intravenous care services are provided.
3.17 Parenteral Nutrition (PN) Nurse
Plan, coordinate and implement the overall PN activities in accordance with the Mission, vision and objectives of the institution in the assigned nursing Units.
Initiate evidence-based approach to the assessment, planning, implementation and evaluation of patient receiving Parenteral Nutrition.
Develop and implement PN administration and protocols to all referred patients.
Identify and initiates an appropriate treatment plan on changes in-patient condition, making safe, timely decisions appropriate to the situation in collaboration with theattending physician, nursing staff and multidisciplinary.
Act as a resource nurse for medical, nursing and other healthcare professionals caring for patients receiving PN.Assist in monitoring hematology, biochemical and microbiology status of patientsreceiving PN.
Assist with assessment and management of complications related to Vascular Access Devices (VAD), e.g. infected VADs or VAD?s occlusion; and other complications associated with PN therapy.
Monitor patients daily by collecting and interpreting patient related data and presenting the information during PN rounds and / or meetings
3.18 Tracheostomy Nurse
Perform initial tracheostomy assessment and provides continuous evaluation of patient care in collaboration with the ENT Consultant and all relevant members of the healthcare team.
Assist in the development and implementation of a hospital Tracheostomy care program to include patient care, protocols, documentation tools and referral criteria.
Initiate evidence-based approach to the assessment, planning, implementation and evaluation of patient receiving Tracheostomy care.
Maintain a collaborative relationship with patients and family emphasizing individualized care, patient autonomy, informed decision making and optimal health care.
Deal with all emergency tracheostomy issues in collaboration with the ENT On call, Medical staff and Respiratory therapist.
Coordinate appointments for urgent and non- urgent cases with various medical teams.
Collaborate with all clinical staff in adherence to standards of care, competencies, policies and procedures in quality and cost effective tracheostomy care.
Make recommendation to the physicians for changes to tracheostomy care and provid evidence based as needed Prepare and maintain reports on a monthly and yearly basis, collect statistics and research
details for the ENT and tracheostomy care committee.
3.19 Endoscopy Nurse
Provide optimum care to patients undergoing special procedures.
Ensure that all equipment are complete and functioning well and ready for use.
Assist the physicians in performing Bronchoscopy, Colonoscopy, Endoscopic Retrograde Cholangiopancreatography (ERCP), Gastroscopy and PEG Placement.
Assist the Anesthetist in giving Sedation to the patient prior to the procedure.
Ensure that patients are stabilized and fully awake from sedation after the procedure before discharging or sending back to the unit.
Ensure that all used equipment are send to Central Sterile Supply Department (CSSD) for sterilization and autoclaving.
3.20 Emergency Room Nurse
Coordinate with all healthcare providers / multidisciplinary team in the implementation Canadian Triage and Acuity Scale (CITAS)
Specialize in assessing, intervening and stabilizing a variety of illnesses and trauma with decisive action according to ED Scope of Service, policy and procedure.
Demonstrate ability to make quick and accurate assessment to all patients including mental health conditions.
Assist physicians for all emergency invasive and non-invasive procedures.
Liaises with other ED personnel to ensure all supplies and equipment are complete and available.
Supervise and support professional development of all nursing staff.
3.21 Obstetrics and Gynaecology Nurse
Coordinate with the Obstetrics and Gynaecology physicians and other Multidisciplinary team in planning and treatment for patient wellness.
Provide appropriate ante-natal, post-natal and gynecology care base on hospital Standards, policy and unit specific protocol.
3.22 Ante-Natal Ward, Obstetric Admission Unit (OAU) and Labor Ward
Assist Staff Midwife in preparing for delivery.
Assist the mother in ambulation and repositioning during labor process.
Conduct an initial nursing obstetric assessment, history taking and triaging of patient upon admission.
Assist in CTG application and monitoring under the supervision of Staff Midwife.
Assist the Staff Midwife when delivering the patient.
Prepare patient for an elective and emergency Caesarean Section.
Assist the Obstetrician and Paediatrician to provide immediate baby care.
Prepare and set-up the suturing trolley and assist during perineal and episiotomy suturing.
Assist the mother in re-positioning.
Care for the new born after delivery :
Weighing the baby.
Measuring the head circumference and length in the presence of a Midwife.
Baby bath and cord care.
Assist during Capillary Blood Glucose testing.
Obtaining Newborn footprints upon delivery.
3.23 Chemotherapy Day Unit (CDU) Oncology Nurse
Thorough health assessment of patients suitability in receiving Chemotherapy as well as validates Chemotherapy prescriptions before sending to Pharmacy.
Provide pre-procedure education of patient and family.
Coordinate with Pharmacy for the availability of Chemotherapy drugs and nonformulary drug.
Monitor and perform the care of the patient during the chemotherapy infusion.
Administers Chemotherapy, Targeted Therapy, Biotherapy and supportive treatment as ordered by the physician.
Identify, manage immediate complications of Chemotherapy, monitor, evaluate and record the treatment results.
Manage and educate patients on Fulfusor Pumps, recognition, early detection and management of complications related to the catheter or infusion device.
Follow safe handling and management of Cytotoxic spill.
Assess and manages CVAD Port Catheters, PICC lines to collect / draw bloods aseptically.
Assist in procedures - IT Chemotherapy, Lumbar Puncture, Abdominal Paracentesis and removal of PICC.
3.24 Adult / Pediatric Hematology / Oncology In-Patient and Day Case Nurse
Administer and monitor conscious sedation under the direction of a physician and documents care such as continuous Morphine administration for the Sickle Cell Anemia patients.
Administer Chemotherapy, Biotherapy, Targeted Therapy and IV medications according to treatment protocols and follows protocols for safe handling of biohazardous materials.
Implement chemotherapy education and other counseling services as needed for patient and family.
Respond to hematology emergency situations and follow department protocols.
Provide psycho-social and emotional support to patient and families.
Work closely with social workers and other cancer wellness staff to coordinate in-depth support and community resources as needed.
Provide health education to patients / relatives to have a better understanding of their health condition, what symptoms may occur, side effects and what methods they can employ to help prevent future occurrences and what treatment options are available.
Assist physician with diagnostic procedure such as Bone Marrow aspiration, biopsy, Central Line insertion, Lumbar Puncture and Intrathecal treatment.
Maintain the care and management of long-term blood access devices.
Prepare patients for the Bone Marrow Transplant and carefull bserving the Stem Cells transfusion for any complications that may occur, provide adequate care of patients post transplant and inform the physician.
Ensure safe administration and management of chemotherapy medication after certification
Assist in liaising with health care professionals to ensure appropriate medical care is provided to all patients.
3.26 Epilepsy Monitoring Unit EMU) Nurse
Provide quality nursing care for patient on Video EEG Monitoring.
Ensure safety of the patient throughout the monitoring period ( padded side rails, wall mounted suction, oxygen, patent IV line, and alarm of the Video EEG must be audible and a functioning call bell.
Evaluate the patient during the seizure attack by checking the speech, comprehension, memory and neurological vital signs.
Respond immediately to seizure alarm to assure patients safety, assist with testing and administer medications when necessary, intervene accordingly and conducts post ictal testing
Inform the Epileptologists in charge after each generalized tonic-clonic seizure for the description / duration of seizure.
Inform the Neurologist to evaluate the patient when necessary.
Document all seizure activity in the patients Medical Record including the date, time of onset, termination, duration & description, pupillary changes and patient orientation after seizure and specific interventions.
3.27 Radioactive Iodine 131 Therapy Nurse
Initiate a comprehensive patient assessment including baseline observation.
Increase awareness of associated risks of exposure and corresponding interventions.
Manage Iodine spill as per hospital policy and procedures.
Educate patient / relatives on the basic principles, purpose and side effects of Radioactive Iodine 131 Therapy.
Ensure the use of correct Personal Protective Equipment (PPE?s) during entire course of therapy.
Document date and time of administration of Radioactive Iodine 131, side effects, patient compliance and tolerance to therapy.
3.28 Operating Theatre Scrub Nurse
Check and prepare the needed instrument sets and supplementary items in the Surgeon's preference card.
Check the sterilization indicators on all instrument sets and supplementary items to be used.
Demonstrate proper scrubbing in adherence to Medical City Wide Policy and Procedure.
Maintain sterility of all surgical fields for the entire procedure / surgery.
Count audibly with the Circulating Nurse all instrument sets, swabs, sharps and supplementary items.
Handover to circulating nurse any surgical specimen / s and after surgery log in the Specimen logbook and ensure that the in Laboratory Department staff collect the specimen / s.
Dispose all instruments, swabs, sharps after the surgery / procedure accordingly.
Ensure that all swabs, instruments, supplementary items from the previous case are removed and the room totally cleaned before the next patient is brought into the Theatre Room.
3.29 Operating Theatre Circulating Nurse
Ensure the cleanliness of the Theatre Room before pushing in the patient.
Check the Safety Checklist before the start of the theatre list.
Transfer the patient from the Holding Area to the Theatre Room with the Main Responsible Surgeon (MRS) and the Main Responsible Anesthetist (MRA).
Assist the patient in transferring from the trolley to the operating table.
Monitor the patient until patient is fully anesthetized.
Assist the Surgeon and Anesthetist in the positioning of the patient ensuring all limbs are supported, padded and secured on the operating table.
Check instruments, swabs, sharps and other supplementary items with Scrub Nurse audibly and record in the White Wipe Board.
Ensure that the correct size of diathermy pad is properly applied to the patient.
Conduct Time-Out before skin incision using the Surgical Safety Checklist.
Provide additional items needed.
Monitor the patient positioning throughout the operative procedure to ensure patient's safety.
Receive surgical specimen / s from the Scrub Nurse.
Monitor any item that drops on the floor from the surgical field and informs the Scrub Nurse.
Clean the patient and observe privacy after surgery.
Check the necessary instrument sets and supplementary items needed for the next surgery.
Handover to the Post Anesthesia Care Unit (PACU) nurse together with the Main Responsible Anesthetist (MRA).
3.30 Post Anesthesia Care Unit (PACU) Nurse
Secure patient airway.
Monitor patient's vital signs.
Administer medication as prescribed.
Check surgical wounds for bleeding and pathological indications.
Assess patient's overall condition during the delicate post anesthesia period.
Educate patient regarding self-care techniques following the procedure. (Outpatient Surgery).
3.31 Angiogram Nurse
Assist the Charge Nurse in planning, coordinating and directing the nursing care of patients.
Maintain good standards of services within the department.
Facilitate in booking Radiological cases according to priorities.
Assist the Radiologists for any interventional procedures.
Attend on emergency on-call cases.
3.32 Rheumatology Day Case Unit
Thorough health assessment of patient's suitability in receiving Chemotherapy as well as validates Chemotherapy prescriptions before sending to Pharmacy.
Provide pre-procedure education of patient and family.
Coordinate with Pharmacy for the availability of Chemotherapy drugs and nonformulary drug.
Monitor and perform the care of the patient during the Chemotherapy infusion.
Administer Chemotherapy, Targeted Therapy, Biotherapy and supportive treatment as ordered by the physician
Identify, manage immediate complications of Chemotherapy, monitor, evaluate and record the treatment results.
Manage and educate patients on Fulfusor Pumps, recognition, early detection and management of complications related to the catheter or infusion device.

3.33 Hemodialysis Nurse
Prepare the delivery system, dialysate, dialyzer and confirms that all mandatory alarm tests on the dialyzer and equipment are performed.
Conduct pre-hemodialysis patient assessment including vital signs, vascular access status, laboratory findings and patient's general health.
Perform hemodialysis pre, intra and post, patient's monitoring, response to treatment, hooting machine as needed and demonstrate an in-depth understanding of the mechanics of dialysis.
Assess and prepare the patient's vascular access for initiation of treatment (cannulation of AVF / AVG, hemodialysis catheter exit site dressing).
Assess patient post-hemodialysis prior discharge, condition of vascular access and document relevant information.
Ensure disassembly and disinfection of dialysis equipment.
3.34 Peritoneal Dialysis Nurse
Assess prospective patient for Peritoneal Dialysis as referred by the physician.
Perform initial flushing of Tenkchoff catheter in the Angiogram right after its insertion to ensure patency.
Evaluate and supervise patient after training, identify needs for re-education and conduct health education during follow-up to ensure safe dialysis at home.
Conduct regular follow-up with the patient thru clinic visits and phone calls. Review the dialysis record to ensure that they are adhering with standards.
Conduct adequacy test at least every 6 months and annual Peritoneal Equilibrium Test.
Refer results to the Physician to identify needs for revision of patient's prescription at home.
Change the Titanium and extension line of the Tenckhoff catheter every 6 months or as needed to prevent infection.
Coordinate with the multidisciplinary team to meet the general needs of the patient in performing dialysis at home (Social Worker, Dietician, Home Support, Logistician).
Attend In-Patients and Out-Patients for Peritoneal Dialysis, trouble shoot machines, provide necessary treatment (independent and dependent) for site infection, peritonitis, malfunctioning Tenckoff catheter as per protocol.
Conduct interviews to know the patient's capability to perform Peritoneal Dialysis at home.
Ensure proper disposal of fluids, effluents and dialyzer.
Disinfect machines and equipment accordingly.
3.35 Burn Unit Nurse
Assesses, plans, coordinates, and implements and evaluates nursing care for plastic and burn patients according to scope of service, policy and procedure of Prince Sultan Military Medical City.
Examines, implements and participate in wound dressing including but not limited to burn and plastic cases and ensure appropriate treatment is provided to the patient in collaboration with multidisciplinary team for optimal wound healing.
Maintain skin graft viability and educate patient and families to promote healing.
Identify changes in the patient's condition that requires immediate attention including but not limited to wound discoloration or hemorrhage and pain management and
coordinate with the physician and multidisciplinary team to ensure adequate provision of patient's needs.
Assess and monitor the patient's emotional and psychological trauma secondary to burn injury in collaboration with the physician and psychiatry / psychology team.
Promotes patient's well being by educating patients and families to understand burn wound conditions, medication and self care skills.
3.35 Post Natal Ward
Care for the baby in the Post-Natal Unit :
Record feeds and nappy changes.
Must do the Newborn Screening (NBS) at 18 hours of life.
To ensure that Newborn Hearing Test is done upon discharge.
Newborn care under Phototherapy.
Assist the mother during breastfeeding.
Educate the mother regarding feeding of the baby.
Securing of Newborn Footprints during discharge.
4.0 Education / Licensure :
4.1 Essential : Bachelor 's Degree in Nursing
4.2 Essential : License :
Current practicing license from country of origin and with current Saudi Council Registration as a Specialist.
4.3 Essential : Certificate Certified BCLS, Sedation Course Certified, ACLS, PALS, NRP and Chemotherapy Course for specialized areas such as Emergency Department, GICU, NICU, PICU, OAU, Labor Ward, Ward 5N, CDU, Rheumatology Day Case, Hemodialysis and Peritoneal Dialysis.
4.4 Essential : Certificate : Post basic certificate in area of responsibility.
5.0 Professional Experience :
